Job Summary
A healthcare company needs applicants for an opening for a Telecommute Senior Registered Nurse Case Manager in Houston.
Core Responsibilities of this position include:
- Engaging members and families in their homes or in a facility to coordinate services and treatment needs
- Delivering a holistic approach to coordinated care based on the member’s needs using a person centered approach
- Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file
Position Requirements Include:
- Travel in assigned territory as needed to complete member assessments
- RN License, must be current and unrestricted in state of Texas
- 3+ years of recent experience as a RN in case management, home health or clinical experience
- 1.5+ years of Medicare and Medicaid Program experience
- High Speed Internet access in home, and must be proficient with Microsoft Word, Outlook, Internet and Excel
- Experience in implementation of training and compliance process improvement activities and action plans
- Access to reliable transportation
- Previous managed care experience
