Job Summary

A healthcare company is searching for a person to fill their position for a Telecommute Senior Social Work Case Manager.

Core Responsibilities Include:

  • Conducting psychosocial assessments to identify individual needs
  • Addressing objectives and goals as identified during assessment
  • Implementing social work goals by advocating for members needs

Applicants must meet the following qualifications:

  • Master's degree is in Social Work
  • Current, unrestricted LCSW or LMSW in applicable state(s)
  • Willingness to obtain licensure in additional states is required
  • 3+ years of clinical experience; or any combination of education and experience
  • 5+ years of Case Management experience
