Job Summary

A healthcare company needs applicants for an opening for a Telecommute Social Services Health Coordinator in the College Station Area.

Candidates will be responsible for the following:

  • Assessing, planning and implementing care strategies that are individualized by patient
  • Identifying and initiating referrals for social service programs
  • Managing the care plan throughout the continuum of care

Required Skills:

  • Ability to travel in the assigned region to visit Medicaid members
  • Active RN license in the state of TX or the ability to obtain upon hire
  • 4+ years of experience working within the community health setting in a health care role
  • Experience in a long-term care, home health, hospice, public health or assisted living environment
  • Pediatric experience
