Job Summary

A well-known healthcare company has a current position open for a Telecommute Social Worker Case Manager in the Green Bay Area.

Candidates will be responsible for the following:

  • Occasionally visiting members in their homes and/or other settings
  • Supporting members with condition education, medication reviews and connections to resources
  • Assisting members with the transition from a care facility back to their home

Must meet the following requirements for consideration:

  • Access to reliable transportation and travel approximately 25%
  • 3 years clinical experience in a hospital, acute care, home health/hospice, direct care or case management
  • Current, unrestricted RN license or Masters of Social Work (MSW)
  • Computer/typing proficiency to enter/retrieve data in electronic clinical records
  • Experience with email, internet research, use of online calendars and other software applications
