Job Summary
A healthcare company is seeking a Telecommute Social Worker in Charleston.
Candidates will be responsible for the following:
- Assist identified members in assessing their care needs
- Assist in planning and implementing interventions to meet those needs
- Connect potential dual-eligible membership to resources using Low Income Subsidy guidelines
Applicants must meet the following qualifications:
- Travel: Yes, 10 % of the Time, Valid driver's license and/or dependable transportation necessary
- Current Social Work license in the state in which the individual will be required to practice
- Minimum of 3 to 5 years experience working as a licensed Social Worker
- Master's Degree in Social Work
- Excellent communication skills, both verbal and written
- Proficient in the use of Microsoft Office products including Word, Excel and Outlook
