Job Summary
A comprehensive healthcare services company is searching for a person to fill their position for a Telecommute Social Worker in Dayton.
Individual must be able to fulfill the following responsibilities:
- Facilitate serious illness / goals of care discussion with the patient and family
- Conduct an assessment of the patient’s and family’s strengths and needs
- Navigate community resources and developing community partnerships to improve access to care and ease of systems navigation
Position Requirements Include:
- Master’s Degree (MSW) in Social Work
- Meet all state certification and licensing requirements (LSW or LISW in OH required)
- Minimum of three years of social work experience (internship not included)
- Minimum of two years of social work experience in a healthcare setting
- Excellent communication skills and knowledge of accessing community resources
