Job Summary
An American managed health care company needs applicants for an opening for a Telecommute Social Worker in Orlando.
Individual must be able to fulfill the following responsibilities:
- Ffocus on quality of care, compliance, outcomes and decreasing costs
- Perform initial and periodic assessments of the members enrolled in the Long Term Care Program
- Apply case management concepts, principals and strategies in the development of an individualized care plan for enrolled members
Qualifications for this position include:
- Complies with all state requirements in the state where job duties are performed
- Minimum of two years experience in medical social work
- Ability to travel locally
- Bachelor's degree in Social Work
