Job Summary

A healthcare company has a current position open for a Telecommute Specialized Recovery Services Community Based Case Manager in Erie County.

Candidates will be responsible for the following:

  • Providing ongoing care planning and case management for individuals with serious mental illness
  • Determining eligibility for the program

Applicants must meet the following qualifications:

  • Requires travel to individual's homes
  • Minimum 3 years experience (post degree) working with persons with severe and persistent mental illness
  • Active license as RN, LSW, or LISW. OR Bachelor's degree in Social Work, Counseling, Psychology or related field
  • Minimum of 1 year post degree experience working with individuals with diagnosed chronic conditions
