Job Summary
A healthcare company has a current position open for a Telecommute Telephonic Case Manager.
Individual must be able to fulfill the following responsibilities:
- Make outbound calls to assess members' current health status
- Deliver clinical support to members across a wide array of health topics and conditions
- Identify gaps or barriers in treatment plans
Must meet the following requirements for consideration:
- Current, unrestricted RN license in the state of employment
- 3+ years of clinical experience in a hospital, acute care setting
- Must have excellent typing skills
- Ability to navigate a Windows based environment
- Certified Case Manager (CCM) or the ability to obtain certification
- Ability to work independently with minimal supervision
