Job Summary

A healthcare company has a current position open for a Telecommute Telephonic Case Manager.

Individual must be able to fulfill the following responsibilities:

  • Make outbound calls to assess members' current health status
  • Deliver clinical support to members across a wide array of health topics and conditions
  • Identify gaps or barriers in treatment plans

Must meet the following requirements for consideration:

  • Current, unrestricted RN license in the state of employment
  • 3+ years of clinical experience in a hospital, acute care setting
  • Must have excellent typing skills
  • Ability to navigate a Windows based environment
  • Certified Case Manager (CCM) or the ability to obtain certification
  • Ability to work independently with minimal supervision
