Job Summary
A healthcare company is filling a position for a Telecommute Telephonic Transitional Care Coordinator.
Core Responsibilities of this position include:
- Making outbound calls and receiving inbound calls to assess members' current health status
- Identifying gaps or barriers in treatment plans
- Providing patient education to assist with self – management
Applicants must meet the following qualifications:
- Current, unrestricted RN license in the state of employment
- 4+ years of experience in a hospital, acute care or direct care setting
- Experience working for long-term care, home health, hospice, public health or assisted living
- 2+ years of experience working with MS Word, Excel, and Outlook
