Job Summary

A managed health care company is in need of a Telecommute Transition Coordinator.

Core Responsibilities Include:

  • Leading care transition planning process
  • Participating in discharge planning for Members
  • Ensuring a safe transition that meets the Member`s needs and preferences

Skills and Requirements Include:

  • Associates degree in Nursing
  • Registered Nurse, State and/or Compact State Licensure - Care Mgmt
  • 1+ years of experience working directly with individuals who meet the MLTSS target population criteria or
  • 1+ years of experience working as a RN
  • Demonstrated ability to communicate with members who have complex medical needs
