Job Summary

A global healthcare company needs applicants for an opening for a Telecommute Transition Coordinator and DCS Liaison in Indianapolis.

Must be able to:

• Oversee day-to-day clinical program operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating)
• Ensure team meets established performance metrics and performance guarantees
• Conduct back up, field-based RN visits as needed in designated markets

Qualifications Include:

• Ability to travel up to 15% on an as needed basis within a designated area
• BSN degree or equivalent experience
• 3+ years of clinical experience in a Indiana Medicaid prior authorization or case management setting coordinating member care
• Current unrestricted RN License in state of Indiana
• Experience in an Indiana Managed Care Organization
