A pediatric hospital is seeking a Telecommute Transplant Health Care Management Associate.

Core Responsibilities Include:

• Helping with the development of a discharge plan
• Assisting the case management staff in pre-admission planning and service set up
• Processing referrals for discharge planning

Position Requirements Include:

• High School Diploma, or equivalent
• 2 years administrative health care experience
• Ability to communicate effectively both orally and in writing
• Ability to provide empathy in difficult interpersonal situations
