Job Summary
A health insurance company needs applicants for an opening for a Telecommute Transplant Nurse Case Manager.
Candidates will be responsible for the following:
- Making outbound calls to assess members' current health status
- Identifying gaps or barriers in treatment plans
- Providing patient education to assist with self-management
Skills and Requirements Include:
- Current, unrestricted RN license in your state of residence
- 3+ years as an RN in a hospital setting, acute care, direct care or experience as a telephonic Case Manager
- 1+ years of transplant experience with solid organ or bone marrow as a Nurse
- Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
- Multiple state licensure or ability to obtain multiple state nursing licenses
- Access to high speed internet from home
