Job Summary

A global healthcare company has a current position open for a Telecommute Transplant RN Case Manager in Las Vegas.

Core Responsibilities Include:

• Collaborating with member, provider, facility to obtain best outcome
• Communicating/Developing/Implementing ongoing plan of care in conjunction with member and providers
• Collaborating with internal members of the health care team, to include PCP, FHS, COC and Medical Director

Position Requirements Include:

• Current and unrestricted RN license in the state of Nevada
• 4+ years case management experience
• Proficient in Microsoft Word, Excel and Outlook
