Job Summary
A healthcare company has an open position for a Telecommute Utilization Management Clinical Care Coordinator.
Core Responsibilities Include:
- Conducting clinical assessments that address the health and wellness needs of members
- Serving as a subject matter expert to both internal and external sources
- Developing/implementing case or condition-specific plans of care and/or intervention plan, as needed
Position Requirements Include:
- Ability to travel locally and work flexible hours in a practice or facility-based settings
- Current PA RN license and/or additional states as required or current Social Work license
- 4 years experience in clinical, case management, disease/condition management, or related
- Clearances as required by specific practice or hospital, as applicable
- Proficiency in MS Excel and enhanced data and statistical analysis skills
- Experience working with physicians in provider practice settings and members in their home
