Job Summary
A healthcare solutions company has an open position for a Telecommute Washington Licensed Healthcare Case Manager.
Individual must be able to fulfill the following responsibilities:
- Engage target members (those who are facing medical, mental health and/or substance utilization disorders)
- Conduct initial holistic health assessments that help build an understanding of a member's specific situation
- Create a space where a case manager and member can collaborate, along with relevant caregivers and providers
Skills and Requirements Include:
- Registered Nurse, ARNP, Registered Dietitian or Master's/Doctoral degree in health or human services related field
- Current state credential (e.g., licensure) that allows the professional to conduct an assessment independently as permitted
- For behavioral health, required licensure (e.g., LMFT, LICSW, LPC, LMHC)
- 3 years' experience with direct patient/clinical care post licensure
- High Speed Cable, DSL or Fiberoptic Internet, no less than 20 mbps
- All Case Managers are required to have valid WA state licensure
