Job Summary

A healthcare company needs applicants for an opening for a Telecommuting Behavioral Health Case Manager in Austin.

Must be able to:

  • Assess and review plan of care regularly to identify gaps in care
  • Conduct in depth health risk assessment and/or comprehensive needs assessment
  • Educate providers, supporting staff, members and families regarding care coordination role

Qualifications for this position include:

  • Must have a state driver's license
  • Masters degree in Mental Health required
  • Independently licensed behavioral health professional
  • Minimum 5 years post masters clinical behavioral health/psychiatric nursing experience required
