Job Summary
A physician-owned, physician-operated group of health plans and related companies has a current position open for a Telecommuting Behavioral Health Community Care Coordinator in Rockford.
Candidates will be responsible for the following:
- Serve as the direct personal contact in the community
- Acts an advocate for the member, assuring they receive the supports and services they desire and are eligible to receive
- Develop and maintain relationships with key individuals in the community and act as an advocate to linkages or referrals to improve health, social, and environmental conditions for MHP members
Qualifications for this position include:
- BA/BS degree in social science, social work or related
- Current license to practice in the designated State is preferred, minimum restricted licensure or evidence of progress to independent licensure status
- Current unrestricted license(s) to practice in the designated State(s)
- One (1) year of supervised social work or counseling experience in a health care setting, working directly with individuals, is required
- Minimum one (1) year experience in discharge planning, home health care, community health or managed care is required
