A staffing company needs applicants for an opening for a Telecommuting Bilingual English Spanish or English Creole Long Term Care Coordinator in Miami Dade County.

Must be able to:

  • Complete assessments with members, caregivers, or providers to obtain information regarding client status
  • Authorize and coordinate referral for services
  • Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care

Position Requirements Include:

  • Must have valid Florida Driver's License
  • Must live in Miami Dade County
  • High School Diploma OR GED AND 6 years of care management experience.
