Job Summary
A healthcare company needs applicants for an opening for a Telecommuting Bilingual Spanish English Social Worker in Palm Beach County .
Candidates will be responsible for the following:
- assist identified members in assessing their care needs
- assist in planning and implementing interventions to meet those needs
- monitoring and evaluating the case management plan against the member's personal health care goals
Qualifications for this position include:
- Travel to onsite facilities up to 25% - as business needs require
- Meet members face to face
- Hours are Monday-Friday 10:30-7:00 PM - as business needs chang
- Bachelor's Degree in Social Work
- Progressive experience in a medical or related setting required
- Possession of a valid driver's license and access to a reliable automobile
