Job Summary
A healthcare company is in need of a Telecommuting Case Manager Remote in New Orleans.
Core Responsibilities of this position include:
- Conducting telephonic assessments with the members to gather information regarding the member's current treatment
- Promoting and providing quality service delivery and effective care transitions planning
- Utilizing the components of the case management process
Skills and Requirements Include:
- Able to travel 20-30% of the time
- Strong clinical and case management skills
- Has a current, unrestricted state license issued by a state or territory of the United States to practice independently as a Clinical Social Worker
- Minimum of 3-5 years of direct clinical care experience
- Strong computer and keyboarding skills
- Valid driver's license and proof of insurance mandatory for positions which require driving
