Job Summary

A healthcare company is searching for a person to fill their position for a Telecommuting Childrens Health and Social Services Coordinator in McAllen.

Candidates will be responsible for the following:

  • Assessing, planning and implementing care strategies that are individualized by patient
  • Identifying and initiating referrals for social service programs
  • Managing the care plan throughout the continuum of care as a single point of contact

Qualifications Include:

  • The ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices
  • Active RN license in the state of Texas or the ability to obtain upon hire
  • 4 years of experience working within the community health setting in a health care role
  • Experience in long-term care, home health, hospice, public health or assisted living
  • Pediatric experience
