Job Summary

A staffing agency is filling a position for a Telecommuting Clinical Case Manager in Downers Grove.

Individual must be able to fulfill the following responsibilities:

• Providing and coordinating community resources and referrals
• Providing member education on disease process
• Working with Medicaid members over the phone and within their homes

Required Skills:

• Will visit members face to face every 90 days - 6 months depending on their acuity level
• RN, LSW, LCSW, LCPC, LPC
• Any other applicable experience
