A healthcare company has an open position for a Telecommuting Community Case Manager.
Candidates will be responsible for the following:
- Assessing, authorizing and providing ongoing case management
- Helping a diverse population of persons with chronic illnesses or disabilities of all types
- Manageing caseloads covering several counties
Skills and Requirements Include:
- Possible field based travel
- Active license as RN, LSW, or LISW
- 12 months experience in home and community based services within the past 5 years
