Job Summary
A healthcare company is in need of a Telecommuting Community Transitions RN Case Manager in Milwaukee.
Core Responsibilities Include:
- Making concerted effort to find members post hospital that are difficult to reach
- Making initial post hospital call within 3 days of discharge to ensure that discharged member receive the necessary services and resources
- Identifying gaps or barriers in treatment plans
Must meet the following requirements for consideration:
- Access to reliable transportation and ability to travel to visit members in the hospital
- Current, unrestricted RN license in Wisconsin
- 5+ years’ experience as an RN in a clinical setting
- Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
- Must have a designated work space and access to install secure high speed Internet via cable / DSL in home
