Job Summary

A healthcare company is in need of a Telecommuting Diabetes Education Community Health Worker in Toledo.

Individual must be able to fulfill the following responsibilities:

• Serve as an intermediary between Health and Social Services
• Facilitate access to health services to remove social, cultural and environmental barriers
• Improve identified health outcomes

Qualifications Include:

• Home-based position that requires travel to individual's homes
• High School Diploma, GED or higher education level
• One-year experience with community resources within area served
• Effective oral and written communication skills and attention to detail
• Proficient computer skills and familiarity with suite of Microsoft Office Programs
