Job Summary
A healthcare company has an open position for a Telecommuting Licensed Practical Transitions Care Nurse in Fort Myers.
Core Responsibilities Include:
- Working with recently discharged patients to prevent health complications, improve patient outcomes, enhance access to quality care, and reduce hospital readmissions
- Providing prompt care coordination for patients during transitions between healthcare settings
- Working closely with the patient, caregiver, physician, RN care coordinator, care team, and acute care facilities to ensure the patient's individual needs are met
Applicants must meet the following qualifications:
- Graduate of an approved Licensed Practical/Vocational Nursing program (Associate's Degree/Diploma)
- Minimum of 3 years of clinical experience with emphasis on patient education, chronic disease care; home health, skilled nursing, or medical-surgical nursing
- Licensed Practical Nurse licensure required
- Proficient computer skills, including Microsoft Office (Word, Excel, and PowerPoint) required
- Knowledge of adult learning principles and education methods required
- Exceptional communication skills and ability to develop and maintain effective relationships with patient, staff and care team require
