Job Summary

A healthcare company is searching for a person to fill their position for a Telecommuting Licensed Social Worker Care Coordinator in Chesapeake.

Individual must be able to fulfill the following responsibilities:

  • Actively participate during care transitions
  • Manage assigned caseload
  • Ensure that appropriate mechanisms are in place to receive enrollee input

Qualifications for this position include:

  • Conduct home visits
  • Must be an LCSW, LMHP, or LPC
  • Bachelors degree in either Human Services, Healthcare or related field
  • Must have at least 1+ years of clinical experience
  • Must be an independently licensed behavioral health clinician
