Job Summary
An insurance company is searching for a person to fill their position for a Telecommuting LTSS Case Manager in Plantation.
Must be able to:
- Be responsible for performing telephonic or face-to-face non-clinical assessments
- Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities
- Be responsible for overall management of member's individual service plan within the scope of position
Applicants must meet the following qualifications:
- Travel required
- May also serve as mentor or preceptor for new staff
- 2 years of experience working with a social work agency; or any combination of education and experience
