Job Summary

A healthcare company is in need of a Telecommuting Medical Case Manager.

Must be able to:

  • Communicate effectively by phone with customers and external contacts
  • Collaborate with Healthcare providers in order to coordinate member care
  • Develop an individualized plan of care designed to meet the specific needs of each member

Required Skills:

  • Attend and participate in required meetings
  • Bachelor’s degree in nursing or RN certification or Master’s degree in Social Work and 3 years experience in Acute or Managed Care/Medicaid or Medicare populations, OR
  • Bachelor’s degree in Social Work with five years experience in Acute or Managed Care/Medicaid or Medicare populations
