Telecommuting Nurse Care Manager II

Job Summary

A health insurance company is filling a position for a Telecommuting Nurse Care Manager II.

Individual must be able to fulfill the following responsibilities:

• Monitor and evaluate effectiveness of the care management plan and modifies as necessary
• Assess the medical necessity of inpatient admissions and extensions of stay
• Collaborate with providers to assess consumer needs for early identification of and proactive planning for discharge

Qualifications Include:

• Requires 3 years of acute care clinical experience
• Current, unrestricted RN license in applicable state(s) required
• Condition-specific clinical experience and home health/discharge planning experience
• At least 1 year in a Nurse Care Mgr I role or equivalent experience
• Multi-state licensure is required if this individual is providing services in multiple states
• Must have knowledge of medical management process
