Job Summary
A healthcare company has an open position for a Telecommuting Patient Review and Care Coordinator in Seattle.
Individual must be able to fulfill the following responsibilities:
- Making outbound calls to assess members' current health status
- Identifying gaps or barriers in treatment plans
- Providing patient education to assist with self management
Applicants must meet the following qualifications:
- Initial in-office orientation and training and some local travel
- 3+ years of experience working in a healthcare environment
- Proficient computer skills including the ability to type and talk at the same time
- Experience managing and prioritizing multiple responsibilities
- Proficient using MS Word, Excel, Access and PowerPoint
