Job Summary

A healthcare company has an open position for a Telecommuting Patient Review and Care Coordinator in Seattle.

Individual must be able to fulfill the following responsibilities:

  • Making outbound calls to assess members' current health status
  • Identifying gaps or barriers in treatment plans
  • Providing patient education to assist with self management

Applicants must meet the following qualifications:

  • Initial in-office orientation and training and some local travel
  • 3+ years of experience working in a healthcare environment
  • Proficient computer skills including the ability to type and talk at the same time
  • Experience managing and prioritizing multiple responsibilities
  • Proficient using MS Word, Excel, Access and PowerPoint
