Job Summary
A business solutions company is searching for a person to fill their position for a Telecommuting Population Health Manager.
Individual must be able to fulfill the following responsibilities:
- Coach on pre-visit planning and closure of gaps in care
- Fortify the collaborative relationship we seek to establish with our physicians
- Enhance access to care and referral network optimization
Skills and Requirements Include:
- Must be able to deliver programs, training, and adoption to client
- Minimum of 3-5 years of experience working with physician practices
- Minimum of 3-5 years of client facing experience within the healthcare domain
- Demonstrated change management expertise
- Extensive knowledge of the U.S. healthcare system
