Job Summary
A healthcare company has a current position open for a Telecommuting Registered Nurse Case Manager in Long Island .
Core Responsibilities of this position include:
- Managing accounts using the 4-Pringes approach
- Determining and documenting participant's goals
- Engaging the PCP and treating physicians in the personal health management process
Must meet the following requirements for consideration:
- Bachelor degree in a health related field
- Five (5) years of UM/CM experience
- Three to five (3-5) years of diverse clinical experience required
- Understanding of IMMS holistic approach to medical management
- Understanding of the client's (customer's) perspective and needs
- Understanding of legislative acts, such as the ADA
