Job Summary
A health insurance company has a current position open for a Telecommuting RN or MSW Field Care Manager in Columbiana County.
Core Responsibilities Include:
- Visiting members in their home and using a compliment of instruments that helps to identify gaps in care
- Developing plan of action with your assigned member based on identified needs
- Evaluating member needs and implementing appropriate interventions with At Home member participants
Qualifications for this position include:
- Must be able to travel 50%
- Possession of valid state driver's license and access to an automobile
- 2 years of experience of in home case/care management
- RN with a valid nursing license or MSW
- Knowledge of community health and social service agencies and additional community resources
- Experience working with the adult population
