Job Summary

A for-profit health insurance company has a current position open for a Telecommuting Social Services Care Manager.

Individual must be able to fulfill the following responsibilities:

• Evaluating member needs and requirements to achieve and or maintain optimal wellness state
• Guiding members and their families towards and facilitate interaction with resources appropriate for the care and wellbeing of members
• Working in collaboration with a multi-disciplinary team, employing a variety of strategies, approaches and techniques to manage member physical, environmental and psycho-social health issues

Must meet the following requirements for consideration:

• Masters degree in Social Work
• 3 years of case management experience with adults
• Licensed in your residential state
