Job Summary

A health insurance company is searching for a person to fill their position for a Telecommuting Social Worker.

Core Responsibilities Include:

  • Case management activities focusing on quality of care, compliance, outcomes and decreasing costs
  • Performing initial and periodic assessments of the members enrolled in the Long-Term Care Program
  • Conducting comprehensive management of members with acute or chronic conditions

Applicants must meet the following qualifications:

  • Ability to travel locally as required
  • Minimum of 2+ years experience in medical social work or case management
  • LMSW/LCSW required
  • Complies with all state requirements in the state where job duties are performed
  • Bachelor's degree or equivalent experience
