Job Summary

A healthcare company has an open position for a Telecommuting Social Worker .

Must be able to:

  • Oversee the comprehensive management of members with acute or chronic conditions
  • Develop and carry out strategies to coordinate and integrate all acute and long-term care services
  • Assist in the identification and reporting of potential quality improvement issues

Qualifications Include:

  • Ability to travel locally as required up to 10% of the time
  • LBSW/BSW
  • Bachelor's degree or equivalent experience
  • Minimum of 2+ years experience in medical social work or case management
  • Ability to comply with all state requirements
