Job Summary

An insurance company has a current position open for a Telecommuting Social Worker in Beachwood.

Core Responsibilities of this position include:

• Developing relationships with hospital social workers and community resources to assure appropriate care management
• Applying case management concepts, principles and strategies in the development of an individualized case plan
• Being responsible for the comprehensive management of members with acute or chronic conditions

Qualifications for this position include:

• Ability to travel locally as required, 10% - 25%
• Nursing/Licensed Social Worker (LCSW) is REQUIRED
• Minimum of 2 years experience in medical social work or case management
• The minimum level of education desired for candidates in this position is a Bachelor's degree or equivalent experience
• Complies with all state requirements in the state where job duties are performed
• 1-3 Years of Customer Service/Functional and Clinical / Medical experience
