Job Summary

A healthcare company has a current position open for a Telecommuting Special Population Case Manager in Dover.

Core Responsibilities of this position include:

  • Utilizing applicable sources of information to identify, assess, and enroll patients
  • Assessing, planning, coordinating, implementing, monitoring, and evaluating the care of each beneficiary
  • Coordinating and collaborating with other members of the Special Populations

Position Requirements Include:

  • Registered Nurse with current in state RN license
  • Certified Case Management
  • 2 years Case Management Experience with emphasis on transplants/clinical trials or complex pediatrics
  • Interqual (or Milliman) knowledge
  • Computer literate                                   
  • Good communication skills, verbal and written
