Job Summary

A provider of care management solutions to reduce costs and improve health is searching for a person to fill their position for a Telecommuting Support Intensity Scale Assessor in Philadelphia.

Candidates will be responsible for the following:

  • Administering a needs assessment instrument to individuals enrolled in the Consolidated and P/FDS waivers, receiving Autism services, services from private or public ICF/IDs, on DHS-ODP’s waiting list and individuals identified to be enrolled in the programs
  • Collects assessment-related data and entering needs assessment data
  • Comply with all applicable regulations and requirements (HIPAA, Medicaid, etc.)

Qualifications for this position include:

  • Must attend monthly SIS team meetings as well as travel as part of the SIS assessor team
  • Experience working directly with individuals diagnosed with intellectual disabilities and Autism Spectrum Disorder
  • Experience interviewing and communicating with individuals with ID and Autism and administering standard assessments
  • Passing criminal background clearances from the Pennsylvania State Police and Child Abuse Clearances per the Child Protective Services Law (“CPL”), 23 Pa. C.S. Chapter 63
  • 4-year college degree
