Job Summary
A healthcare company is filling a position for a Telecommuting Telephonic Health and Social Services Licensed Vocational Nurse.
Must be able to:
- Engage members / families telephonically to coordinate services, community resources and treatment needs
- Deliver a holistic approach to coordinated care based on the member’s needs using a person centered philosophy
- Identify early risk factors and conducting ongoing assessments and documenting in an electronic file
Applicants must meet the following qualifications:
- Must be able to commute to Texas facility for training and meetings
- LPN/LVN, Certified Nursing Assistant and/or social worker
- HS Diploma
- 3+ years of experience working within the community health setting in a health care role
- Experience in long-term care, home health, hospice, public health or assisted living
- 2 + years of experience working with MS Word, Excel and Outlook
