Telehealth Care Coordinator

Job Summary

A company is looking for a Telehealth Care Coordinator to support clients throughout their treatment journey.

Key Responsibilities

• Conduct comprehensive telehealth intake assessments and coordinate care with internal teams
• Provide education and support to clients, monitoring progress and addressing barriers to care
• Maintain accurate documentation and ensure compliance with organizational policies and regulations

Qualifications

• Associate's degree or higher in Social Work, Psychology, or a related Human Services field
• Two to three years of experience in behavioral health, mental health, or substance use disorder treatment settings
• Adherence to the Healthcare Code of Ethics and professional standards of conduct
• Knowledge of state and federal confidentiality regulations and Joint Commission standards
• Ability to work effectively as part of a multidisciplinary treatment team
