Telehealth Care Coordinator

Job Summary

Supporting clients throughout their treatment journey, the full-time Telehealth Care Coordinator will manage client intakes, care coordination, and communication with multidisciplinary teams while working remotely.

Key Responsibilities

• Conduct comprehensive telehealth intake assessments and coordinate with internal teams for smooth treatment initiation
• Provide education and support to clients, conducting check-ins to monitor progress and reinforce treatment goals
• Maintain accurate documentation and ensure compliance with organizational policies and regulatory standards

Required Qualifications

• Associate's degree or higher in Social Work, Psychology, or a related Human Services field
• Two to three years of experience in behavioral health, mental health, or substance use disorder treatment settings
• Knowledge of state and federal confidentiality regulations and Joint Commission standards
• Adherence to the Healthcare Code of Ethics and professional standards of conduct
• Ability to work effectively as part of a multidisciplinary treatment team
