Job Summary

A company is looking for a Telehealth Case Manager Nurse.

Key Responsibilities

• Conduct comprehensive health assessments and gather relevant medical, social, and lifestyle information
• Provide telephonic and digital education, nursing interventions, and coaching on health topics
• Coordinate with healthcare providers and community resources to facilitate access to necessary services

Required Qualifications, Training, and Education

• Registered Nurse (RN) licensure with a minimum of 3 years of clinical experience
• Experience in health management, care coordination, or telephonic nursing is preferred
• Associated degree or Diploma in Nursing; Bachelor's preferred
• Must obtain CCM Certification within 4 years of employment
• Ability to be licensed in all non-compact states, territories, and the District of Columbia as needed
