Job Summary

A company is looking for a Case Manager - Registered Nurse - Telehealth.

Key Responsibilities

• Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs
• Utilize clinical tools to conduct evaluations and apply clinical judgment to address health and social indicators
• Collaborate with supervisors and stakeholders to overcome barriers and enhance member wellness

Required Qualifications

• Registered Nurse with an unrestricted nursing license and multi-state/compact privileges
• 3+ years of clinical/nursing practice experience
• 1+ year of medical case management, care coordination, or discharge planning experience
• High-speed internet access and proficiency in MS Office Suite applications
• Associate's degree in nursing required; BSN preferred
