Job Summary
Seeking a compassionate and mission-driven Telehealth Nurse Practitioner - FNP, CNP, the full-time remote position will conduct virtual patient visits, assess symptoms, and create effective care plans while focusing on serving underserved populations.
Key responsibilities
- Provide telehealth care to patients through the NeverAlone program, emphasizing service to underserved populations
- Perform thorough assessments and collaborate with care teams to ensure continuity of care
- Document all patient encounters accurately and in a timely manner
Required qualifications
- Active Nurse Practitioner license in Maryland, West Virginia, Virginia, Ohio, or Indiana
- DEA license required
- 3-5+ years of experience as a Nurse Practitioner
- Family NP certification
- Experience in acute, post-acute, nursing home, palliative care, and/or ED settings preferred
