Job Summary

A company is looking for a Telehealth Primary Care Physician.

Key Responsibilities:

• Learn Solace systems and tools, prioritize patients' needs, and oversee patient care plans
• Identify and maintain streamlined care continuum for patients
• Collaborate with healthcare advocates to holistically address social determinants of health

Required Qualifications:

• Board certification in family medicine or internal medicine with a strong interest in geriatrics
• Minimum 2 years of experience or recent gerontology fellowship completion
• 10 to 15 unrestricted state licenses and/or active member of an IMLC state
• Valid DEA certification
• Technical savvy and ability to quickly learn new systems and software
