Key Responsibilities

• Provide patient education and support for medication adherence and disease management through telephonic outreach
• Develop, implement, and evaluate comprehensive care plans for patients with various disease states
• Collaborate with healthcare professionals to enhance the patient experience and ensure continuity of care

Required Qualifications

• Unrestricted Registered Nurse license in the state of residence with multi-state/compact privileges
• Must be based in a compact state
• At least 3 years of clinical RN experience
• Proficient in Microsoft Office applications, including Word, Excel, and Outlook
• RN Diploma, Associate's, or Bachelor's degree in Nursing
